Development and preliminary testing of the new five-level version of EQ-5D (EQ-5D-5L)

The new five-level version of the EQ-5D (EQ-5D-5L) was developed by the EuroQol Group to improve the instrument's sensitivity and reduce ceiling effects. The EQ-5D currently uses three levels in five dimensions to assess health status. The new version increases the number of severity levels to five in each dimension, resulting in the EQ-5D-5L. The study was conducted in the UK and Spain, with focus groups used to evaluate the face and content validity of the new versions. The results showed that labels at the 25th, 50th, and 75th percentiles were preferred, with a slight preference for the wording 'slight-moderate-severe' in the functional dimensions. The new version was tested in focus groups, and participants found it easy to understand and complete. The EQ-5D-5L was found to have good face validity and initial content validity. However, further testing is needed to determine whether the new version improves sensitivity and reduces ceiling effects. The study also highlights the importance of consistency in label selection across dimensions and the need for further psychometric testing to validate the new version. The EQ-5D-5L is now available in UK English and Spanish for Spain, with translations planned for use in 25 additional countries. The new version is expected to have better discriminative capacity and sensitivity to change than the EQ-5D-3L, as well as smaller ceiling effects.
