Management of Rheumatoid Arthritis in Primary Care: A Scoping Review

This scoping review examines the challenges and strategies in managing rheumatoid arthritis (RA) in primary care (PC). The review analyzed 61 articles published between 2013 and 2023, focusing on critical issues in RA management in PC, such as delayed diagnosis, difficulty in accessing rheumatology care, ineffective communication between GPs and specialists, poor patient education, lack of cardiovascular prevention, and increased healthcare costs. Strategies to address these issues include early diagnosis, quick access to rheumatology care, effective communication, active patient involvement, screening for risk factors and comorbidities, clinical audit, interdisciplinary patient management, digital health, and cost analysis. PC is considered the ideal setting for reducing the morbidity and mortality of chronic diseases, including RA, if GPs adopt a more proactive approach. The review highlights the importance of early diagnosis, effective communication, and the use of digital health tools in improving RA management. It also emphasizes the need for improved cardiovascular disease prevention, better coordination between PC and rheumatology, and the implementation of clinical audit to ensure adherence to guidelines.
